First Visit
Your initial visit is a crucial step in the treatment planning process. We will review your health history, perform an examination, and explain your diagnosis. Your consultation visit will conclude with a brief visit with one of our financial coordinators who will review the proposed treatment plan and associated fees, and schedule a convenient time for your surgery.
Occasionally, surgery may be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide your surgical treatment .
For same-day surgery, patient must be prepared — you may not eat or drink anything for six (6) hours prior to your appointment and you must have an adult drive you home and stay at home with you. Please read our Before Anesthesia Instruction.
Please assist us by providing the following at your first visit:
- Surgical Referral slip from your referring dentist/doctor
- A clear, readable, diagnostic, and current panoramic X-ray only, if taken by your dentist
- If covered by a DHMO plan, you must bring an insurance referral from your general dentist, your appointment will be rescheduled without it.
- A list of all medications you are presently taking
- Picture ID
- Your dental AND medical insurance cards
Patients under 18 years of age must be accompanied by a parent or legal guardian for all appointments.
